Free Google Forms exposes patient data on the first submission. Even paid Workspace leaves structural compliance gaps that no configuration can close.
Our product team will walk you through your specific setup in a working session. See exactly how your forms flow into patient records, what access control looks like for your roles, and how to go live.

HIPAA requires a set of baseline technical safeguards for any platform handling protected health information. Here is where each tool stands on each one.
Google Forms dumps every submission into a shared sheet with no access controls, no record change logs, and no way to restrict what your staff can see.
Knack Health gives you the same drag-and-drop simplicity with a signed BAA, field-level permissions, and a full record change log on every submission — all inside one HIPAA-ready platform.
Knack Health integrates directly with your existing EHR and practice management stack. Patient intake data flows automatically into Epic, Athenahealth, ModMed, and 700+ more — no manual exports, no copy-paste, no compliance gaps in between.
Launching is easy.
Pre-built and HIPAA-ready from day one. Pick a template, customize your fields, set role-based access, and go live — no developer, no compliance project, no waiting.

Only under specific conditions. Free Google Forms is never HIPAA compliant. On a paid Google Workspace plan, you can sign a BAA through the Admin Console, but structural gaps remain: there are no field-level access controls, no record change logs, and no automatic data retention enforcement. A signed BAA alone does not make a platform compliant.
Knack Health includes a signed BAA on every HIPAA plan, encryption at rest and in transit, field-level role-based access controls, and full record change logs on every record. Compliance is built into the platform by default, not configured on top of it.
No. A BAA is only available on paid Google Workspace plans. Free Google accounts have no BAA option, meaning any patient data collected through a free Google Form is an unprotected disclosure with no legal framework governing how Google stores or handles it.
A record change log tracks who accessed a patient record, when, what they viewed, and what they changed. HIPAA requires this to ensure accountability and to support breach investigations. Google Forms records submission timestamps only. It does not log which staff member opened a specific response or when.
Knack Health HIPAA plans start at $159 per month with no per-user fees. Google Workspace, which is required before any BAA is available, starts at around $6 per user per month and still does not include field-level access controls or record change logs at lower tiers.
Not safely on a free account, and only with significant configuration on a paid Workspace plan. Even then, every staff member with sheet access sees every patient response with no way to restrict fields by role. For a compliant patient intake workflow, you need field-level access controls, a signed BAA, and record change logs — none of which Google Forms provides out of the box.
Many forms can go live within a day. Knack Health includes pre-built templates for patient intake, consent forms, referral and transfer forms, and screening questionnaires. The BAA is included on every plan so there is no separate compliance setup or IT project required before you can start collecting patient data.
